DSWD AICS - Medical Assistance
Requirements for AICS Medical Assistance
DSWD Medical Assistance provides financial support to individuals and families facing medical emergencies who cannot afford necessary healthcare expenses. It aims to ensure that vulnerable and marginalized sectors have access to essential medical services.
Assistance ranges from ₱5,000 to ₱75,000 in justifiable cases and may cover hospitalization, medication, laboratory tests, specialized procedures, and other related medical costs.
Requirements
- Valid ID of the person applying or being interviewed
- Authorization Letter (if applicable)
- Medical Certificate/ Clinical Abstract/ Discharge Summary/ Alagang Pinoy Tagubilin Form issued within three months (Original or Certified True Copy)

Bring the following documents based on the type of assistance being requested:
Hospital Bills:
- Hospital Bill or Statement of Account (Outstanding Balance) with the name and signature of the billing clerk
- or Certificate of Balance and Promissory Note signed by the credit and collection officer/billing clerk
- Social Case Study Report or Case Summary
Medicine or Assistive Devices:
Prescription with date of issuance, patient’s complete name, license number, and signature of the physician, issued within three months
If requesting assistance above ₱10,000, the following additional documents are required:
- Quotation of Medicine or Assistive Device
- Social Case Study Report or Case Summary
Laboratory or Medical Procedure:
- Laboratory Request/ Laboratory Protocol/ Doctor’s Order with name, license number, and signature of the physician
If requesting assistance above ₱10,000, the following additional documents are required:
- Quotation of Laboratory
- Social Case Study Report or Case Summary
